The COLOURS organization is a program partner of the Urban Affairs Coalition (UAC).

The Health & Wellness Coordinator is responsible for overseeing the delivery and integration of educational, developmental, and supportive services that address the social determinants of health within our programs. Then will collaborate with internal teams and external partners to develop culturally responsive health strategies that expand access to behavioral health support, substance use treatment, HIV prevention, and outpatient medical care. In addition, this role will ensure clients are connected to vital supportive services to include rental assistance and housing stabilization resources- to help individuals maintain long-term housing and reduce homelessness risk. Working closely with the Community Engagement Team, the incumbent will support the planning and implementation of events and initiatives that promote client recruitment, retention, engagement and access to care. These efforts will include referral and follow-up for HIV testing, STD screening, PrEP services, medical and behavioral care, and other wraparound services that enhance the well-being of vulnerable populations. The role will also encompass a range of administrative duties to ensure accurate service. Below is an overview of various administrative duties:

MAJOR RESPONSIBILITIES:

  • Coordinate clinical and community partnerships to deliver navigation and case management services that ensure timely linkage and follow-up to medical care, including HIV treatment, mental health services, and biomedical interventions such as PrEP, PEP, TasP, and condom use—as well as support for housing stabilization and long-term retention in care.
  • Coordinate and implement health-focused programs and activities that promote mental and physical well-being, with an emphasis on reaching individuals at heightened risk for HIV.
  • Deliver individualized education and self-management resources tailored to each client’s language, cognitive ability, literacy level, and stage of readiness for behavioral change.
  • Provide daily oversight of community engagement activities and service delivery processes to ensure timely, high-quality, and client-centered care.
  • Facilitate timely and appropriate referrals to medical, behavioral health, and social service providers to support comprehensive care.
  • Actively collaborate with designated partner organizations to ensure coordinated service delivery and optimize client outcomes.
  • Design and implement health and wellness programs with clearly defined, measurable outcomes to evaluate impact and inform continuous improvement.
  • Identify, evaluate, and provide ongoing oversight and support to all COLOURS Program Facilitators to ensure effective program delivery, participant engagement, and alignment with organizational goals and standards.
  • Develop and/or curate culturally relevant educational materials for distribution, including content tailored for social media platforms.
  • Deliver HIV prevention and general health education through interactive presentations and community outreach activities.
  • Proactively recruit and engage individuals through outreach efforts to connect them with supportive services offered by COLOURS.
  • Ensure all clients are linked to appropriate, high-quality social, medical, and supportive services to promote HIV prevention, treatment adherence, and overall health.
  • Promote safer sex practices and harm reduction strategies, particularly among individuals who use drugs or engage in high-risk behaviors.
  • Maintain accurate and timely documentation of all services provided and prepare reports in compliance with agency policies and management directives.
  • Comply with OSHA regulations and safety protocols, including infection control and exposure prevention related to bloodborne pathogens and hazardous materials.
  • Conduct individualized sessions and facilitate group presentations to promote health education and behavior change.
  • Perform additional duties as assigned by management to support the mission and operations of the organization.

QUALIFICATIONS:

  • One to Three years of experience
  • Preferred candidate - Bachelor's or higher degree in Education, Public Health, Social Services, or related fields.
  • Knowledge of HIV prevention and Harm Reduction strategies required.
  • Some clinical experience with HIV prevention and harm reduction strategies
  • Demonstrated excellent oral and written communication skills.
  • Demonstrates understanding of the cultural and ethnic diversity of sexual and gender minority people of color.
  • Ability to multi-task.
  • Willingness to travel to partnering agencies, client appointments, boards and committees that support program outcomes.
  • Provide individualized and group presentations.
  • Willing to work evening weekends, Flexibility is a must.
