As a full time Medical Social Worker you will be an integral part of the care coordination team, working in collaboration with RN case management, the clinical team, and internal/external collaterals. As a member of our interdisciplinary team you will be responsible for performing psychosocial assessments, providing supportive counseling and care coordination with respect to discharge planning and service coordination for adult, geriatric, pediatric, and med/surg populations. Experience with protective services, child welfare, long term care, crisis intervention, chronic illness, hospice care, substance use disorders and psychiatric illnesses is a plus. Schedule is 40 hours day shift with weekend and holiday coverage required, schedules given months in advance.
