The Licensed Practical Nurse (LPN) provides direct nursing care and support to patients under the supervision of registered nurses and physicians in a Medical Group setting. Responsibilities include gathering patient data, preparing patients for treatments, assisting with examinations, and supporting patient self-care and management. The role demands adherence to state nursing regulations, ongoing education, quality improvement, and effective communication within the healthcare team.
The Licensed Practical Nurse (LPN) provides general nursing care to patients, families and caregivers in the Medical Group practices. The Licensed Practical Nurse (LPN) assists and advocates for the patient and families health in support of the patient centered approach and delivery model of care, and is an active care team member. Responsible for collaborating with physicians/ providers , registered professional nurses and multidisciplinary care team members. A Licensed Practical Nurse (LPN) can perform tasks and responsibilities within the framework of case finding, health coaching, and supportive care under the direction of the registered professional nurse or licensed physician or licensed health care provider. Nursing shall perform in accordance with the standards outlined for Licensed Practical Nurse (LPN) in the NYS Education Law, NYS Office of the Professions, and by the State Nurse Practice Act. A Licensed Practical Nurse will follow the mission, vision, I-CARE values, policies and procedures of Mohawk Valley Health System.
Required:
Graduate of an accredited school of licensed practical nursing program
Preferred:
Bachelors or Masters of Science in Nursing from accredited nursing program
