Case Manager (On-Site – Silver Lake, CA)

Location: Silver Lake, CA

Schedule: Monday–Friday, 8:30 AM – 5:00 PM

We are seeking a compassionate and organized Case Manager to join our team at a newly launched property management site in Silver Lake. This role is part of a broader initiative to support residents transitioning out of homelessness through wraparound services, including housing stabilization, behavioral health, and community integration.

As a Case Manager, you will provide on-site intensive case management to medium- to high-acuity households. You’ll work closely with property management and community partners to ensure residents receive the support they need to thrive.

Key Responsibilities

  • Develop and implement individualized service plans (ISPs) using trauma-informed and harm-reduction approaches.
  • Provide support in areas such as independent living skills, budgeting, employment linkage, and health care coordination.
  • Conduct risk assessments and intervene in crisis situations.
  • Maintain accurate documentation in case management systems and comply with reporting requirements.
  • Collaborate with property management to prevent evictions and promote housing stability.
  • Advocate for residents to secure necessary resources and services.

Qualifications

  • Experience in affordable housing, homeless services, or behavioral health strongly preferred.
  • Strong interpersonal and communication skills.
  • Ability to manage competing priorities and work independently.
  • Familiarity with Housing First principles and trauma-informed care.
  • Proficiency in Microsoft Office and case management software.
  • Bilingual (Spanish/English) a plus.
